Limited spaces. Each program and age group has limited capacity. If your child’s group is full, they’ll be placed on the waitlist for that activity and we’ll contact you when a spot opens.

5 Health information

This helps our coaches and volunteers keep your child safe. Write “None” if nothing applies.

6 Fees and payment

The registration fee is $300 per child for the October to June season (soccer, basketball or both). Summer is free. We’ll confirm your child’s spot and payment details by email.

7 Agreements

I understand that sports and physical activities involve a risk of injury, and I give permission for my child to take part in DYES programs, games and events. In an emergency, if I can’t be reached, I give permission for DYES staff and volunteers to get medical treatment for my child. My child and I agree to follow the DYES code of conduct and show respect to coaches, volunteers, teammates and opponents. The information on this form is correct, and I’ll tell DYES if anything changes.
